Reflective practice makes you think about what you are doing and how you do it, according to Blackburn(1999) but Fulmer (1993) states that reflective practice begins with the examination of an individuals own actions. It can result in behavioural changes that improve professional performance. The clinical skill I will focus on will be Electrocardiograph (ECG) recording, which is a non-invasive procedure that is used to ascertain information about the electrophysiology of the heart as stated by The Royal Marsden (2004). According to Norfolk and Waveney Mental Health NHS trust (2007), some people on Antipsychotic medication can have side effects that can have affect on the hearts rhythm and so these clients require regular ECG recordings. This assignment will be written in the first person and shall refer to myself as “I” Hamil (1999) recommends writing in the first person when using reflection. I propose to use the Gibbs (1998) reflective cycle as it makes you look at you’re feelings and it makes you evaluate the way you think about things. My ward manager and mentor both agreed that I should be able to carry out 12 lead ECG’S as part of my expanded roll. As I would be able to put into practice and use on a regular basis and would be beneficial for the clients and aid my colleagues. The skill needed to be one that I could carry out unsupervised after an initial period of time. Learning agreements were devised as shown in appendix 1, this states what I need to learn, strategies and resources I could use to learn this skill and an evaluation in which identifies how I will know I have met my learning objectives such as accurately completed documentation. 2 I attended a study day which was arranged by my practice educator where I would gain the basic knowledge of the anatomy and physiology of the heart. I also gained an understanding of the National Service Framework For Coronary Heart Disease, DOH(2000) which established standards for prevent and treatment in which 12 lead ECG’S are considered an effective intervention. Within my clinical area I had also arranged to observe medical staff carrying out EGC’S as shown in appendix 2. I would gain the practical knowledge of electrode placement due to there being two types of leads which are unipolar and bipolar as stated by Net doctor (2007). I also took part in a distance e-learning course which covered areas of the 12 lead ECG in acute coronary syndrome as shown in appendix 3 along with a final exam as shown in appendix 4. The medical staff would be the ones to supervise my initial first ECG within my clinical area. My thoughts and feelings went through many changes as I made the journey through the learning process. I had no confidence in myself when starting to learn the physiology and anatomy of the heart and what all of the peaks and dips mean on an ECG recording. As my knowledge improved my confidence grew, asking the medical staff questions helped as they would put the answer into context of the client’s within my clinical area. I am know at the stage that I am competent in explaining the procedure to the client and know am proficient in carrying out 12 lead ECG’S when needed for the continuous client care. On reflection of the whole learning process from my manager stating that being able to carry out 12lead ECG’S should be part of my expanded roll unto I actually carried out my first unsupervised ECG within my clinical area. My skill level had to go through many changes from being a novice to being a competent practitioner. My performance had good points at different stages of the learning process, at the beginning due to the therapeutic relationship I had with the clients it made them feel I knew what I was doing 5 clinic’s within the trust where I am employed. If at any time I am unable to carry ECG’S and feel that I need to observe and have a supervised ECG I would arrange with the medical staff on my own clinical area or other clinical area.
