Ano-rectal Suppuration * Definition: Infection in the ano-rectal spaces. * Aetiology: A) Predisposing factors: 1. Infected anal gland is the commonest cause (90%). 2.Lack of personal hygiene. 3.Infected hair follicle, sebaceous cyst, perianal haematoma, or fissure. 4. Injection treatment of piles. 5. Spread of a pelvic infection eg. Appendicitis , salpingitis ...etc 6.On top of anal fistula — recurrent ano-retal suppurations. 7.Rarely inflammatory bowel disease, T.B. & anorectal carcinoma. Terminal radicles of sup. Coli, B. proteus, strept ... etc. C) Route of infection: 1. Usually infection spread from the lumen of the anal canal or the rectum. 2. Spread may occur from near by infected organ. * Pathology: The infection starts in the ana/ crypts > anal gland — intersphincteric plane where an abscess is formed — this abscess spread in various directions producing: Longitudinal m. Pelvirectal space abscess internal sphincter m. Intersphincteric space abscess: Levator ani m. External sphincter m.
