BOS3525 Corrective Actions for Apparent OSHA Violations Columbia Southern University BOS3525-15G-6A21-S1, Legal Aspects of Safety and Health Memorandum 8 June 2021 RE: Corrective Actions TO: All employees Effective: Immediately As a result of the recent inspection by the Occupational Health and Safety Administration (OSHA), several infractions have come to our attention that need to be immediately addressed and corrected. It is because of these violations that the following corrective actions will be implemented effective from this day forward: 1. Although controls are in place in accordance with CFR 1926.353 (OSHA, n.d.-d) for proper ventilation in the welding shop, based on the amount of haze present during the walk-through, these controls are not being properly utilized. All personnel authorized admittance to the shop shall be trained on the use and maintenance of available equipment and will be responsible for ensuring use during working hours. 2. No welding operations will be performed near flammable materials in accordance with CFR 1626.352(c) (OSHA, n.d.-c), and the presence of such materials should be brought to the attention of the shop foreman and removed before welding is resumed. 3. Employees who display facial hair will not utilize tight-fitting respirators in accordance with CFR 1910.134 (OSHA, n.d.-a). Partial seal respirators are available and will be utilized by these individuals. 4. Eyewash stations will be added to every workshop that utilize injurious corrosive materials without delay in accordance with CFR 1910.151(c) (OSHA, n.d.-b) and are required to be maintained and tested weekly for cleanliness and proper operation. Further, signs will be posted identifying these stations. In addition to these engineering controls, the following administrative controls will be reviewed for updates and additions: 1. Employee training appears to be lacking. Every employee should be trained on company Standard Operating Procedures (SOP) prior to commencing with assigned positions. Along with the controls available above, as well as those forthcoming, Personal Protective Equipment (PPE) in compliance with OSHA standards is available to all. For this reason, weekly toolbox talks will commence every Monday before start of business and will include available equipment, proper use and fitting, proper operation of controls and equipment, and review of SOP. Records will include sign in sheets at every session to verify attendance and document acknowledgement of training. Failure to comply with the standing orders and regulations after acknowledging training will result in corrective actions for the violators up to termination for repeated offenses. Failing to properly utilize safety equipment and follow safety protocol will not be tolerated. 2. All shop foreman will be required to attend OSHA 10-hour training online, and training will be provided to you by the company free of cost. You are expected to educate your subordinates on the training you have been provided and correct any discrepancies in your zone immediately. 3. Safety Data Sheets (SDS) will be maintained for all chemicals regardless of location in a binder and maintained by each shop foreman. Employee’s will be trained on reading and interpreting these sheets, have access to, and always know the location of them. Sheets will be reviewed and acknowledged by signature by every employee no more than every 90 days, or each time a new substance requiring an SDS is introduced to the company. 4. The lock-out/tag-out program will be covered by the SOP and individual procedure binders will be in each zone to be reviewed and signed-off by every employee no more than every 90 days.
