A case review will be outlined, and the problems, goals, and interventions of the case will be recognized. Both the social work skills I demonstrate at the agency and the skills not demonstrated will be explained, as well as the importance of case presentations. Unity Health Unity Health is a nonprofit, community, and teaching hospital. This is a short-term acute care facility. It is in Searcy, Arkansas. Our mission is to improve the quality of health and wellbeing for the communities we serve through compassion care. As a hospital, the patient’s medical issues are prioritized. Some of the services Unity Health offers are primary care, specialty care, inpatient rehab, acute care, pediatric care, surgery, outpatient procedures, maternity care and much more. Behavioral Health Services are also [Shortened Title up to 50 Characters] 2 provided. Unity Health has an inpatient behavioral health campus to address mental health and offers a New Vision program, which is a detox program for substance abusers, and is conducted on the main hospital campus. Unity Health is a member of the Mayo Clinic Care Network which gives Unity Health prime access to all Mayo clinical resources, knowledge, and expertise (Services, 2021). Social services are also available to patients. A social worker’s role is to assist patients on their journey to wellness. This is done in multiple ways such as helping and encouraging a patient to implement the necessary changes in lifestyle, healthcare, diet, and their environment. Social workers may also address a patients mental and emotional health, recommend effective resources and services, and create interventions that help patients adjust to medical diagnoses. This can include setting up home health services or referring the patient to necessary agencies (Hospital Social Worker: Duties, Education, Skills & Salary, 2021). Presenting Case/Patient Demographics1 In this case the patient is a 34-year-old Caucasian female. The patient has several issues. She is Type 1 diabetic and has developed chronic kidney disease after years of not adhering to her medical treatment plans. She also frequently has seizures due to her lack of medical adherence. The patient suffers from both Bipolar 1 disorder and Multiple personality disorder. She is not married but has a dysfunctional relationship with her child’s father. She has an 11- year-old son, who has major cardiology issues. The patient is of a low socioeconomic status, disabled, and a Medicare recipient. This patient has frequent hospital admissions, roughly two a month. [Shortened Title up to 50 Characters] 5 At Unity Health I have not had the ability to engage, assess, intervene, and evaluate groups, although I have demonstrated these skills with families, I am not thoroughly competent with these skills within a group. In practice I do demonstrate cultural awareness and I have engaged in diversity and difference in practice, however at Unity Health I have not worked with a patient that challenged me in this area. For the most part I service a specific demographic of white, rural, and elderly patients in which I am very familiar with due to my experience in a nearby hospice agency. I have not engaged in policy practice outside the agency. I follow agency policy, but I have yet to utilize my advocacy skills to improve policies related to health care or to advance human rights, social economics, or environmental justice. Proposed Professional Development Plan. My proposed professional development plan first began by identifying my desired career path (clinical private practice social work). Next, I identified the skills I will need for my career path. I then analyzed my field competency and identified my strengths and areas that need improvement. To improve my social work skills, I have conducted research and enrolled in certifications that are specific to therapeutic social work. I have researched information regarding licensure in my state and the requirements needed to have a private practice. I am using my field experience to also help with the development of my professional plan by engaging with supervision and asking for feedback on my social work skills. To further improve my skills, I challenge myself with increased responsibility, more difficult job assignments and improved job performance (Professional Development and Training - Social Work Career Center, 2021). The Importance of Case Presentations. Case presentations are an effective tool in professional development and learning. Case presentations allow individuals to practice effective communication skills through delivering [Shortened Title up to 50 Characters] 6 case information. Case presentations also allow an individual to practice for real life experiences in the field. Case presentations are used often in many agencies, and through practice an individual can sharpen up on their communication skills and confidence. Case presentations also provide the opportunity for collaborations or sharing ideas, which can further enhance learning and competence.
