Child Life AAP statement - chid life services should be considered an essential component of quality pediatric health care and are integral to family-centered care and best practice models of health care delivery for children who is a child life specialist? - someone who works in a hospital/other medical settings who focuses on the psychosocial aspect of hospitalization and uses play and other things as tools to help reduce stress and enable patients and families in coping with the health care experience CL history - -1920s and 1930s there were early hospital play programs -founder of child like was Emma Plank -association for the care of children in hospitals was started in 1967 -Child Life Council was founded in 1982 to establish more guidelines APIE - -CLS go through this process to come up with the correct intervention for each individual patient and family -Assessment, Planning, Intervention, and Evaluation play and normalcy - play is used by children as a means of reducing anxiety produced by stressful conditions, medical play or health care play allows children to cope with feelings and exert control over their experiences psychological preparation - preparing children for hospitalizations, clinic visits, surgeries, and diagnostic procedures, helps reduce emotional disturbance, stress, and anxiety, enables the child to plan and rehearse coping strategies key elements of prep - 1. developmentally appropriate info 2. encouragement of question and emotional expression 3. formation of a trusting relationship with a health care professional family support - child life involves education and support of family members, including siblings, helps to facilitate family coping with the adjustment to the child's illness, siblings need info and support to comprehend illness via therapeutic play and education health care environment - ongoing effort to justify the cost of child life services, child life can help transition children back home, school and into the community, there are support groups, camps, hospice, child care and other important things children and families need to cope and regain their lives play - play can help meet children ongoing developmental needs, help children cope with the unfamiliar hospital environment increase children understanding of their hospitalization and treatment, promote a sene of control, mystery, and positive self concept, facilitate self-expression, and meet children's need to cope with separation and deprivation social development of play - children explore potential friendships through play, gain interaction skills, cooperation, problem solving skills, ETC... how can a CLS use play - functional play, constructive play, symbolic play, games with rules, can help with stress, anxiety and other issues the child is having, used for developing, competence skills, helps deal with behaviors and concerns true play criteria - -intrinsic motivation -orientation to means rather than ends -internal rather than external locus of control -noninstrumental rather than instrumental actions -freedom from externally imposed rules and expectations -active engagement toys for infants - crib mobiles, mirrors, cardboard books, rattles, balls, nesting and stacking toys, music box, teething rings toys for toddlers - push and pull toys, riding toys, small blocks, nesting blocks, shape sorters, toy cars toys for pre-school - dress up, housekeeping materials, dolls, toy cars, books, construction paper and crayons, balls, puzzles, music makers, miniature people, board games, play kitchen toys for school age - construction paper and crayons, puppets, word games, card or board games, organized group games, play-doh, dolls, puzzles, advanced art projects toys for teens - arts and crafts, journals, games (w peers), movies, music, puzzles hannahs gift legacy - hospital: holding baby, wearing shoes during surgery, asking doctors names, made doctors do their job differently stress refers to an ongoing transaction between a person and the environment that results in an emotional appraisal that the situation is threatening or harmful to some important aspect of well- being. two aspects of stress: event that causes stress and the individuals response. coping - what a person does to remove, avoid, minimize or get through a stressful situation problem-focused coping - something you can change in the environment emotion focused coping - something you have to change within yourself information processing - being able to take in info, receiving, assimilating, sorting, recalling and applying information. CL may have to help transmit that info to children in a way they will understand. psychosocial support - -supportive time before during and after big events -communication -respect -accurate info and reassurance -empathetic and nonjudgmental -sincere interest in the childs welfare -engaging the child in play -provide support and info to family members music therapy - an established healthcare profession that uses music to address the physical, cognitive social and emotional needs of individuals goals of music therapy - to reduce stress, alter mood, sustain attention, foster family communication, exercise, preparation for event, enhancing social support music therapy vs CL - both us play and other instruments to help children reduce stress and gain normalcy. have the same goals and outcomes expressive play - enabling children to express feelings and energy in an acceptable pleasurable, and physical manner. (coloring, bubbles, throwing paper, etc) familiarization play - explore, manipulate, and play with health related objects in a non-threatening, pleasurable context dramatic play/medical play - spontaneous and guided role play that focuses on health care themes and often includes medical equipment. allows children to explore and play with equipment at their own pace, post procedural and prep play vicarious play - playing for the child, some children are too ill, injured, immobilized, or emotionally traumatized to actually become engaged in play themselves. play with puppets, family figures, personalized dolls, creative art activities with window painting, familiarization and health care play. psychosocial issues related to caring for children - providing kids with truthful answers, being honest with them, knowing the plan of their treatment, keeping them informed, helping them play, prepping them levels of stress assessment - level 1: children are assessed at being low risk, stable medical conditions, well functioning families, react well to stress level 2: behaviors may be different than level 1, longer hospital stays, parents are concerned or stressed, CL provide developmental appropriate play in interactive play rooms level 3: patients with chronic diagnosis's, families and kids used to hospitals, receiving adequate support, cope well to current treatments, going to have numerous future treatments, CL time should be spent with the child level 4: potential for emotionally challenged, past negative health care experiences, unpredictable events happen to these kids, CL should spend a lot of time with kids and family level 5: trauma, ER, high risk children, unstable homes, parents may not be present, experience life threatening traumas, extended care, intensive rehab health care variables for stress - diagnoses, anticipated treatment, number of times in hospital, personalities of health care teams family variables for stress - how available the family is to the kids, support systems, what needs the family has outside the hospital, religious beliefs child variables for stress - age developmental level, responses to procedure AAA (animal assisted actives) - provide opportunities for motivational, educational, recreational, and/or therapeutic benefits to enhance quality of life, specific treatment goals are not planned AAT (animal assisted therapy) - a goal directed intervention in which an animal that meets criteria is an integral part of the treatment process delta society - not for profit organization and leader in human animal bond research (AAA & AAT) certified needs and benefits - needs:a team of administrators, medical and nursing staff, infection control officer, volunteer department, housekeeping, child life, recreation therapy, and public relations. create a mission statement (short and long term goals) and policy benefits: research shows that relationships with animals have a positive influence on health, interacting with pets can lowers blood pressure, reduce stress, decrease loneliness and enhance feelings of well being, animal assisted therapy allows patients to experience the joy of a visiting pet, either their own or a dog certified for hospital visits
