STEP THREE On the Account Activation page, enter the First Name, Last Name, Date of Birth, and the Last four digits/EMPLID I.D from your Personal Information sheet. When finished click OK. • Note: • On this page you are writing the information from the Personal Information sheet. • If you were assigned an “External I.D or Dummy Number”* you can change the assigned number to your correct Social Security (SS) Number by completing a Change of Document Personal Information Form (CDPIF). The form should be completed and returned with the official documentation to Jay Express Services; an appointment may be required. To download and print a copy of the form, visit the College Now website at http://www.jjay.cuny.edu/college-now. • *a dummy number begins with 666-16 or 816-01. • Date of birth should be entered in the mm/dd/yyyy (e.g., 01/05/2005) STEP FOUR • On the Challenge Questions and Answers page, select/create 5 different questions and answers.
