He presents after a fall at his basketball game with injury to the right ankle. Patient states that he went to take a shot and tripped over another player’s foot and fell with other players falling on his right ankle. He states that the pain is worsening; 10/10, with worsening swelling and bruising. The treatments that he has been using are ice, elevation, ace wrap and ibuprofen. On assessment of the right ankle there was a moderate amount of swelling and bruising, circumferential swelling but medial and posterior side of right ankle was most impressive and extends to the anterior tibia/fibula sparing the dorsum, significant tenderness of the right medial malleolus and posteriorly less severe distal tibia and fibula, bilateral pedal pulses were equal and decreased range of motion of the right ankle and unable to bear weight on that ankle related to pain. The x-ray showed a Salter Harris type 2 fracture of the right ankle. The diagnosis based on patient presentation, assessment and imaging is fracture. Part of this health promotion strategy is patient education on the importance of elevation of the ankle for this diagnosis. Elevating the ankle will have numerous benefits for the healing process and recovery from a Salter Harris type 2 fracture. This component of the treatment plan is an effective way to reduce swelling (Seo et al., 2020). A main complaint from this patient was that the swelling continued to increase after his injury, resulting 2022). The use of crutches will have a significant impact on the patient’s recovery and providing proper education will ensure that recovery is not jeopardized by improper use. With the patient’s age and activity level an important part of coping with this injury is to enable him to be as functional as possible. The use of crutches ensures that he can remain mobile and active with proper use of the device (Warees et al., 2022). Again to address the DEI part of this strategy educational information must be provided in a language and educational level that the patient and his family understands. Another consideration is the cost of equipment and insurance coverage that plays a role with the use of this equipment. The safety and quality portion of this plan is the responsibility of the patient and family to use the resources and check that the patient is using the equipment correctly so he does not cause further harm or injury. A psychosocial consideration is to provide resources to help the patient adjust to the change in his activity and mobility level for the time he is in the cast (4-6 weeks). The resources would provide information on safe activities for the patient to be involved in that will not impair recovery. Each of these elements of the treatment plan are important to ensure that the crutches are used appropriately and that recovery is not delayed. Educating the patient and family on cast care is another important health promotion strategy. Patient education of cast care is essential to ensure safety during this time. Orthopedic casting safety is strongly influenced by proper cast care and patient education (Ebnezar and John, 2018). It is the duty of the healthcare professional to provide verbal and written instructions to reduce the risk of cast associated injuries (Mohammed et al., 2019). There are various instructions that this patient and his family need to be aware of. They should be educated on monitoring neurovascular status, maintaining dryness, positioning, hygiene, skin care, pain management, signs and symptoms to monitor for and when to seek medical attention (Mohammed et al., 2019). Evidence-based practice shows that patient education on cast care is necessary to prevent complications (Ebnezar and John, 2018). All of these instructions are essential for proper cast care. For DEI inclusion the materials must be given in a language and education level that the patient and his family understands to ensure that the instructions are followed. The quality and safety considerations for this plan involve the education on neurovascular checks daily and adequate skin care to prevent complications. Psychosocial considerations again focus on resources to help the patient adjust to their temporary change in activity and mobility level.
