Not the right thing for every client Not a cure-all for all client resistance and ambivalence Clients motivation X therapist skills- outcome Not a substitute for clinical judgment and training IN MOTIVATIONAL INTERVIEWING: Motivation is elicited from the client and not imposed from without- Client centered-Listing -Versus Telling –Respectful It is the client’s task, no the provider’s to articulate and resolve ambivalence- Reflect Direct persuasion is not an effective method for resolving ambivalence- Collaborative Partnership THERAPIST INFLUENCE CLIENT MOTIVATION Client drop-out, compliance and outcome are powerfully influenced by therapist characteristics and environment Direct confrontation yields poorer compliance and outcome Simple actions decrease drop-out Empathic therapists have better outcomes WHY MOTIVATIONAL INTERVIEWING? 80% of substance abusers are in precontemplation or contemplation Define success by movement from one stage to the next WE tend to polarize ambivalence t change: Client argues why not to change and Provider argues reasons why to change In motivational interviewing, the client should be making the argument for change WHAT IS MOTIVATION? “Motivation can be understood not as something that one has. But as something that one does. It involves recognizing a problem, searching for a way to change, and then beginning and sticking with that change strategy” Miller & Rollnick State of mind, not a character flaw Multidimensional, dynamic and fluctuating Interactive and modifiable by clinician THE RIGHTING REFLEX An individuals desire to set things right-particularly pralent in thise in the helping progession Self-perception theory” As I hear myself talk, I learn what I believe. People talk themselves into or out of things.
