NR 327 short comment on this scenario - A 42 year old male who is a long time patient with our practice with a medical history to include hypertension, hyperlipidemia, breast cancer, RA, lower back pain, and insomnia. Medications include Ambien, Simvastatin, Clonidine, oxycodone, warfarin, Lasix, and Methotrexate. He reports today with redness and swelling in his left leg and foot. Upon my assessment of the patient the system alerted me to a recent ED visit for a fall at his home. I was able to see that this was just 3 days ago and x-ray and labs were completed. Due to the lab results (elevated uric acid) I was able to diagnose gout. I ordered Probenecid for the gout, as I entered it into the EMR I immediately got an alert that there was a serious drug interaction with one of her current meds. The patient's symptoms and recent medical events lead to a diagnosis of gout. However, the initial treatment plan had to be adjusted due to a potential drug interaction. Medical History and Medications The patient's medical history includes:  Hypertension  Hyperlipidemia  Breast cancer  Rheumatoid arthritis (RA)  Lower back pain  Insomnia The patient is on several medications:  Ambien (for insomnia)  Simvastatin (for hyperlipidemia)  Clonidine (for hypertension)  Oxycodone (for pain)  Warfarin (for blood clot prevention)  Lasix (for fluid retention)  Methotrexate (for RA and cancer) Current Issue and Diagnosis The patient reported with redness and swelling in his left leg and foot. A recent emergency department (ED) visit due to a fall at home was noted. Lab results from this visit showed elevated uric acid levels, leading to a diagnosis of gout. Treatment Plan and Drug Interaction The initial treatment plan was to prescribe Probenecid for gout. However, upon entering this into the Electronic Medical Record (EMR), an alert was triggered
