The interview happened after an emergency during a staff member's lunch break. The staff member noted that a patient's systolic blood pressure dropped over 30 points. This was documented in the patient's electronic chart, with no intervention noted. The staff member did not give other staff a handoff report but left the clinic area for a lunch break. During the staff member's lunch break, the patient became unresponsive, and the blood pressure dropped a substantial amount. Had a handoff report been utilized, other staff, including the charge nurse, would have been aware of the need to monitor the patient closely. The interview consisted of reviewing what happened in this specific situation and taking notes. There were also chart audits performed to find out if other similar events had happened previously. Staff was asked to explain how they relay to other staff that they will be leaving the clinical area for lunch or other breaks. A series of questions regarding the handoff of care were asked to gather a plethora of information about the usual process of staff leaving the clinical area. Staff was allowed to answer questions one on one with the manager in a non- judging environment. The goal was only to gather information to determine why the event occurred and if it could have been preventable. Evidence-Based Interdisciplinary Approach “Miscommunication between care providers can be a contributing factor for errors.” (McGrath et al.,2020) Evidence shows that errors can be caused when communication is lacking, as in the case of the interview. In a study about handover or handoff reporting, nurses were asked to explain their perception of the handover process. (Pun, 2021) This is important because the researchers need to know if the nurses understand the expectations. This is relevant to the situation of the interview because the staff had many different perspectives of the expectations of the handoff reporting as they left for their lunch break. Some staff did not know that a handoff report is required. Learning the staff's perspective proved to be a vital step toward knowing where to educate the staff. This information is helpful when there is a need to make changes for patient safety. Change Theory and Leadership Strategies In an article about communication and SBAR reporting, the authors mention Lewin’s change theory, which is frequently used in nursing practice. (McGrath et al., 2020) Lewin’s change theory consists of 3 steps: unfreezing, change, and refreezing. Unfreezing is when the change is introduced, change is when changes are implemented, and refreezing is sustaining the change. (McGrath et al., 2020) Lewin's change theory could be applied to the situation in the interview if a change is implemented. Lewin's change theory is credible because it has been used successfully throughout many changes made in nursing practice. Handoff reporting is relevant because the three steps can be applied to initiate, change, and sustain a new handoff reporting system. Leaders can use the information gathered in the interview to decide the length of time and amount of education needed to implement the change theory. Collaborative Approach Practice development process is a strategy used in a 2017 study on collaboration and teamwork.
