Brianna and I worked together at a long-term skilled nursing facility. Brianna is a registered nurse and works at the facility as a charge registered nurse. The charge RN is responsible for implementing physician orders, administering medications, performing patient assessments, reviewing patient history, notifying the physician of patient condition change, and other duties. Brianna has been a registered nurse for five years and has experience working on interdisciplinary teams. She states her overall personal experience was positive and practical. The team’s general attitude remained positive and geared towards providing the patient the best comprehensive care. We discussed issues within the organization. The point of most importance was medication errors. Medication errors are common within the facility due to the fast-paced environment and constant distractions. Brianna stated that there had been at least one medication error during medication pass daily over the past month. Medication pass begins when the nurse opens the medication carts to pull medications and ends when the drug is administered and charted. The facility has an electronic health records system in place; however, the system does not have a barcode scanner for verifying the correct medication is pulled. All medicines for the patients on the hall are organized by patient name in the cart. During medication pass, it is typical for the patients to be gone from their room to physical therapy or other appointments, which causes delays in medications being administered on time. There is a constant rush to complete tasks due to the workload. The nurse to patient ratio is typically 1 to 15 at this particular facility. This situation alone places additional stress on the nurse. On average, a patient takes at least eight medications during the morning medication pass.
