Contains content in one or more of the following knowledge of Ethical and legal considerations in the practice of clinical supervision, Theoretical models of clinical supervision, Clinical supervision intervention methods and modalities, Research in clinical supervision 3. Meets all requirements 4. As of the July 1, 2008 to June 30, 2009 renewal period, the three clock hours of counseling supervision continuing education must be accrued in a face-to-face setting. Continuing education may also be accrued when the LPC teaches in programs such as - ✅✅institutes, seminars, workshops, and conferences provided that such teaching is not required as part of the LPC`s regular employment. ___ hours of continuing education are credited for each hour taught. No more than __ hours of continuing education may be accrued per year through teaching activities. An official continuing education validation form furnished by the presenter All licenses issued by the Board shall remain the property of - ✅✅the Board After having fulfilled all requirements for licensure the Board shall mail notification to the licensee, at last known address, of qualification for licensure; and when the license fee is received and the Board approves the candidate for licensure, the license will be - ✅✅mailed to the licensee The Board shall replace a license that is lost, damaged, or is in need of revision upon written request and payment of the license replacement fee. Requests must include the LPCs - ✅✅original license or be accompanied by the damaged license, if available Each LPC is responsible for renewing the license and specialty designation before - ✅✅the expiration date The renewal date of the original license shall be 2 years from the last day of the month in which the license was - ✅✅originally issued After the first 2 years of licensure, the licensee shall submit verification of at least ___ hours of continuing education required for renewal. - ✅✅40 ___________ can be prorated. - ✅✅Fees and CEUs After the initial 2 year licensing period plus the interim period, renewals shall expire each _____, with a renewal fee of $80.00 and a continuing education requirement of 20 hours. - ✅✅June 30th Regardless of the date the Board granted a specialty designation, the date of a specialty renewal shall be the same date as that of the - ✅✅licensure renewal Requirements for renewal include: - ✅✅1. Submission of Continuing Education on the Continuing A current license verification card shall be readily available ________ at any time counseling services are being conducted. - ✅✅on the LPC's person When a license is placed on inactive status, the license and active verification cards shall be - ✅✅returned to the Board The Board shall mail to licensee's last known address, at least ______ prior to the expiration date of the LPCs license, a notice of expiration. - ✅✅45 days The Board shall provide a directory of - ✅✅Licensed Professional Counselors The directory of LPCs shall include but not be limited to the - ✅✅name, academic degree under which the license is held, preferred mailing address, telephone number, and license number of current licensees. The Board shall provide information of consumer interest which describes - ✅✅the regulatory functions of the Board and its procedures to handle and resolve consumer complaints. 3. Outline the terms of a proposed Stipulation, Agreed Settlement, or Consent Order for the informal disposition of the RFI to be submitted to the Board for approval 4. Authorize the filing of a Formal Complaint and Notice of Hearing in order to initiate an Individual Proceeding 5. Close the RFI If the Complaint Committee decides to meet with an individual, the Complaint Committee shall proceed by: - ✅✅1. Notifying the LPC or Candidate that a RFI has been received, outlining the nature of the inquiry 2. Requesting a prompt letter to the Board responding to the merits of the RFI and a meeting with the Complaint Committee for an interview at a date and time determined by the Complaint Committee 3. If the LPC or candidate fails to respond with a prompt letter or fails to meet with the Complaint Committee at their request, the Complaint Committee may refer the RFI for investigation If the Complaint Committee decides to forward the RFI for investigation, the Complaint Committee shall proceed by: - ✅✅1. Contacting an investigator designated by the Board to address the RFI 2. Providing the designated investigator with the name and address of the complainant, if available, and the LPC or candidate 3. Notifying the LPC or candidate by certified mail that a RFI has been received and an investigation is being conducted The Complaint Committee may request that LPC or candidate be interviewed by the designated investigator or by the - ✅✅Assistant Attorney General in a setting under oath. The Complaint Committee may refer the RFI to a law enforcement agency for criminal investigation, if the Committee determines that - ✅✅a crime has or may have been committed. LPCs and Candidates shall cooperate when Board staff, Complaint Committee members, and/or investigators make inquiries concerning a RFI made against them. Failure to cooperate is grounds for - ✅✅further disciplinary action under the Act and/or Rules. Members of the Board shall serve at the pleasure of and may be removed from office by the - ✅✅appointing authority. Members of the Board shall continue to serve until - ✅✅their successors are appointed. Any vacancy of the Board shall be filled in the same manner as the - ✅✅original appointments. Four members of the Board shall constitute - ✅✅a quorum. The Board shall meet at least twice a year, but no more than 4 times a year and shall elect a - ✅✅chair and a vice-chair from among its members. The Board shall only meet as required for: - ✅✅1. election of officers 2. establishment of meeting dates and times 3. rule development 4. review and recommendation 5. adoption of nonbinding resolutions to the Board concerning matters brought before the Board. Special meetings may be called by the - ✅✅chair or by concurrence of any three members.
