(a) Describe the main features and content of a Safety Statement, indicating in your answer what you think is the most critical part of the content. (12) (b) From a practical point of view (i.e. day to day safety management and control), why is the Safety Statement an important instrument/ document? (8) 2. Outline (with the help of a diagram) the main elements of an effective Safety/Risk Management System and describe their main function or purpose. (20) 3. (a) A Material Safety Data Sheet (MSDS) is required by law and by international standards to contain various sections? Indicate or list the main MSDS sections and, in each case, briefly outline the required section content. (15) (b) As a Safety Manager, what precautions or reservations would you keep in mind when examining the content of an MSDS for a new substance being brought into and used in your company? (5) 4. (a) What are the primary factors that govern or dictate the toxicity of a substance to the human? (8) (b) Describe (with the aid of a simple diagram) the main features and structures of the human skin and indicate how, and in what form, substances may enter the body through this route. (12) 5. In the investigation of serious accidents or incidents, it is important for the investigator to be familiar with various theories of how accidents can occur. The Hale & Hale model is one such theory of accident causation, at the level of the individual, providing a framework by which it may be possible to understand how an error was committed, leading to an accident. With the aid of a diagram, outline the over-riding principle and elements of this theory/model, and your understanding of it. (20)
