License # 2277 STUDENT CLINICAL POLICIES AGREEMENT PART A 1. Clinical hours are a non-paid educational experience. A student may not request payment for clinical hours and may not count work hours as clinical hours; however, college credit will be awarded for successful completion. _____ ​Initial 2. It is the responsibility of the student to make the necessary adjustments in personal schedules to assure attendance at the clinical site at the assigned time. _____ ​Initial 3. Capscare Academy for Healthcare Education assigned holidays and breaks apply to all clinical days. If the school is closed for holiday or break, students may not attend clinical hours. ​_____ Initial 4. A student may not refuse a clinical site; however, a clinical site may refuse a student. ​______ Initial 5. Students MUST be in school uniform at all times to include: the school ID badge and other program specific items (see program handbook). ​______ Initial 6. Attendance in clinical is mandatory and all program attendance policies are to be followed. (Please refer to your program handbook). Students must notify the appropriate program official in the event of absence or tardiness per program policy. _____​__ Initial 7. All required timesheets (hours logs) must be turned in to the Clinical Coordinator by the Monday following the previous week. ​_______ Initial 8. Students must successfully complete all required competencies and documents in a timely fashion to successfully complete the clinical experience​. _______ Initial 9. Always remember you are a "guest" in the clinical facility. Therefore, policies, procedures and guidelines pertinent to the role of the student should be read prior to the first day at that site and must be adhered to at all times​. _______ Initial 10. Arrive and depart as scheduled. Prior to leaving an assigned area, students must communicate their departure to their specified supervisors​. _______ Initial 11. License # 2277 12. Ask for clarification from the faculty member or designated supervisor/preceptor if a directive, policy or procedure is not understood. ​______ Initial 13. Arrive at the clinical site prepared to carry out the assignment. Students must not exceed the boundaries of the assignment unless approval has been issued by the faculty member or preceptor/supervisor. ​___ Initial 14. Always be courteous and tactful​. ______ Initial 15. Focus on the needs of the patient rather than on your own​. ______ Initial 16. Be responsible for your decisions. Therefore, students need to question any verbal or written communication that is inconsistent with their education or that is not understood​. ______ Initial 17. Demonstrate integrity by monitoring the accuracy of your own behavior and immediately to the faculty member or supervisor​. ______ Initial 18. Do no harm to patients or their families either by acts of commission or omission​. ______ Initial 19. No cell phones at ANY time during clinical hours while in the clinical site​. _______ Initial 20. No jewelry allowed; only a watch with a second hand​. _______ Initial Part B 1. No artificial nails. Nails must be short and natural colors​. ______ Initial 2. No facial hair or beard accepted. ​_______ Initial 3. No colored jackets accepted; ONLY white, if possible with the school logo. ​______ Initial 4. No tight pants. ​______ Initial 5. Underwear must be conservative in cut and color. No prints. ​______ Initial 6. No visible tattoos at any time. Tattoos must be covered. ​_______ Initial 7. Students must abide by ALL rules in the program handbook in addition to the above. ____ Initial
