After 8 hours of giving effective nursing interventions : Client will have a negative stool culture. Client will pass soft, formed stool no more than 3 x a day. 1. Ask the client about a recent history of: -Drinking contaminated water. - Eating food inadequately cooked. Ingestion of unpasteurized dairy products. 2.Evaluate pattern of defecation. 3. Assess for abdominal pain, abdominal cramping, hyperactive bowel sounds, frequency, urgency, and loose stools. Eating contaminated foods or drinking contaminated water may predispose the client to intestinal infection. Goal met: After 8 hours of giving effective nursing interventions : Client have a negative stool culture. Client pass soft, formed stool no more than 2x a day. SCIENTIFIC BASIS Diarrhea or frequent passing of loose, watery stool is not really a disease but a condition due to underlying factors ASSESSMENT DIAGNOSIS PLANNING INTERVENTION RATIONALE EVALUATION SUBJECTIVE: “permi siya luya tas kapoy ug dagway”, as verbalized by the Mother of ther patient. Measure client weight. Monitor the client’s food intake. Refer to a dietitian if indicated. This will accurately monitor the response to therapy. To determine the amount of food that is consumed. Collaboration with the dietician in order to guide the client about proper nutrition. Goal met: After 8 hours of giving effective nursing interventions client have an increased nutritional intake and able to ingest food. SCIENTIFIC BASIS Inability to absorb or metabolize foods. Inability to procure adequate amounts of food. Increased metabolic needs caused by disease
